Immunization/Physical Requirements

IMMUNIZATION/PHYSICAL EXAMINATION REQUIREMENTS

Please find listed below the immunization and physical examination requirements for admission to the nursing program. The immunization requirements will be due upon acceptance to the Nursing program, prior to the start of classes.

  1. Physical - routine (The Physical Examination Form is to be completed by the Examining Provider. DO NOT schedule your physical exam at this time. Once you have been accepted to the nursing program, you will be advised of the deadline to submit the completed physical form.)

  2. Rubeola (Hard Measles): Immunity to Rubeola must be demonstrated in one of the following ways:
    1. Had Rubeola that is confirmed by physician's office record
    2. Lab report documenting adequate immune titer
    3. Immunized twice with measles vaccine

  3. Mumps: Full immunity to mumps must be demonstrated in one of the following ways:
    1. Had mumps confirmed by physician's office record
    2. Lab report documenting adequate immune titer
    3. Immunized twice with mumps vaccine

  4. MMR (Measles/Mumps/Rubella): Immunized twice with MMR vaccine. NOTE: this will fulfill the requirements for Rubeola and Mumps (see #2 and #3 above).

  5. Rubella (German Measles): Submit lab report documenting adequate immune titer. NOTE: An adequate serum titer is the ONLY acceptable documentation of Rubella immunity. If the titer is negative, you must receive an additional Rubella vaccination.

  6. Chicken Pox: Full immunity to Chicken Pox must be demonstrated in one of the following ways:
    1. Had Chicken Pox confirmed by physician record
    2. Lab report documenting adequate immune titer
    3. Immunized twice with Chicken Pox vaccine

  7. Tetanus/Diphtheria/Pertussis: Full immunity to Tetanus/Diphtheria/Pertussis must be demonstrated. One Tdap immunization (as an adult) must be administered followed by a Td booster every 10 years.
    1. Documentation of Tetanus/Diphtheria/Pertussis immunization - one time dose as an adult -OR-
    2. Documentation of Tetanus/Diphtheria immunization within ten years of Tdap

  8. Tuberculosis: Absence of active Tuberculosis is required and must be documented in either one of two ways:
    1. PPD (Mantoux) 2-Step TB test. Directions: Two PPD (Mantoux) skin tests need to be performed at least 7 days apart (and no more than 21 days apart) with documentation of each result. Each TB test requires two visits as each test must be read 48-72 hours after it is placed.
    2. Chest x-ray within the last 3 years

  9. Hepatitis: Hepatitis B immunity must be demonstrated in either one of two ways:
    1. Lab report documenting adequate immune titer
    2. Documentation of at least the first of three immunizations upon program admission, and timely documentation of each subsequent dose as they come due.

  10. Flu Vaccine: All health career students are required to receive an annual flu vaccination (Sept. - Nov.)
